The tummy skin moves past it as it is pulled down. A new hole is made in the skin over the now covered belly button to let it out. This creates a small scar around it. If you have a lot of extra skin, the hole representing the former home of your belly button can be pulled down enough to be removed. If you have less extra skin, this may not be possible. In this case, the hole is stitched closed, leaving a small scar. Not a big deal, especially when you consider the overall improvement.
